Midshaft clavicle fractures: What is the optimal treatment in displaced fractures and are radiological indicators for surgery reliable?

The best treatment for patients with clavicle fractures has yet not been clarified. The potential for healing of a clavicle fracture is great whether it is treated conservatively or surgically. Because of this conservative treatment has historically been the preferred treatment and is still to be considered as "the gold standard", regardless of fracture type. In recent years the functional outcome after clavicle fractures has been in focus. Probably because of this there has been a major shift towards primary operative treatment of displaced midshaft clavicle fractures. This has occured despite lack of compelling evidence for superior results.

With this project we wish to clarify what role surgical intervention with locking plate osteosynthesis has in the primary treatment of displaced midshaft clavicle fractures.

The project consist of four studies with following objectives:

1: Investigate whether there is a significant improvement in functional outcome in patients with a displaced midshaft clavicle fracture treated with a locking plate compared with conservative treatment.

And to report the incidence of different surgical outcomes and complications.

2: Investigate whether the shortening of a clavicle fracture judged by radiographs varies with different radiographic tilts. What imaging method is the most optimal when the complexity (fragmentation and shortening) of the fracture is assessed?

3: Validate the Constant score and the DASH score to displaced midshaft clavicle fractures.

4: Investigate what treatment for displaced midshaft clavicle fractures is offered at the different Orthopedics departments all over Scandinavia
