Abstract
This case report describes an 88-year-old male with severe tophaceous gouty arthritis, diabetes, and kidney impairment. Over 20 years, the patient experienced recurrent gout attacks and developed tophaceous deposits, some of which ulcerated. Walking was severely impaired by foot deformities, pain, and chronic foot ulcers. Despite multiple consultations and radiological evidence of gout, no urate-lowering therapy was prescribed. Finally, microscopic analysis of foot ulcer debris identified urate crystals, confirming gout. The patient was then started on allopurinol therapy.
