The present study seeks to investigate neurocognitive, neuroanatomical, cellular and epigenetic markers in MZ twin-pairs discordant or concordant for affective disorder. Previous research has indicated that each of these areas could reveal potential endophenotypes and/or biomarkers in patients with affective disorders. Limited success in the search for susceptibility genes for affective disorder has led to increased attention to endophenotypes, which may pave the way for new insights into the aetiology of these disorders and aid earlier detection and correct treatment. Investigation of monozygoic (MZ) twins also provides a particularly strong methodology in endophenotype research, and to this end, the Danish Registers provide a unique recruitment platform. The results of this study will delineate candidate endophenotypes that are either common for UD and BD, or unique for each disorder, exploring the association between endophenotypes at each level and whether the severity of endophenotypic changes is related to genetic illness-load. It is our hope that this research will advance our understanding of key pathophysiological mechanisms in the etiology of affective disorders and, in longer-term, aid earlier diagnosis and treatment.
